BUSHIDO TATTOO CONSENT FORM

I have read, fully understood and agree to the terms and conditions below. I understand and accept that there are no refunds available under any circumstances.

In signing the form above I agree to the following terms and conditions laid out by Bushido Tattoo

• I have been fully informed of the inherent risks associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks that may arise from tattooing.

• The Artist and Bushido Tattoo will give me instructions on the care of my tattoo while it's healing, and I understand them and will follow them. I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me. If any touch-up work to the tattoo is needed due to me own negligence, I agree that the work will be done at my own expense. I remove any responsibility from the Artist or Bushido Tattoo should I contract an infection.

• I understand and accept that no refunds for tattoos will be given under any circumstances.

• Neither the Artist nor Bushido Tattoo is responsible for the meaning or spelling or the symbol or text that I have provided to them. I will have the chance to check any spellings before the tattoo begins and if I fail to correct the Artist on any changes I desire I will not hold the Artist responsible.

• Variations in colour and design may exist between the tattoo art I have selected and the actual tattoo when it is applied to my body. I also understand that over time, the colours and clarity of my tattoo will fade due to unprotected exposure to the sun and naturally occurring dispersion of pigment under the skin.

• I acknowledge that it is not reasonably possible for the Artist to determine whether I may have an allergic reaction to the dyes, pigments, materials or processes used in completing my tattoo.

• To waive and release to the fullest extent permitted by law each of the Artist and Bushido Tattoo from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from application of the tattoo, whether caused by the negligence or fault of either the Artist or Bushido Tattoo or otherwise.

• I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed by the Artist without duress or coercion.

• That both the Artist and Bushido Tattoo have given me the full opportunity to ask any and all questions have been answered to my total satisfaction.

• I acknowledge that I have been given adequate opportunity to read and understand this document and I understand that I am signing a legal document waiving certain rights to recover against the Artist and Bushido Tattoo.

• Bushido Tattoo treats all personal information lawfully and correctly and operates in accordance with the EU General Data Protection Regulation (GDPR). We make all reasonable efforts to ensure personal information:

*is processed lawfully

*is adequate, relevant and not excessive

*is accurate and, where necessary, kept up to date

*is not kept longer than necessary

*is processed correctly

*is kept secure

*will not be disclosed, distributed or resold to any third parties
